Pack your device and all required items securely.
Affix the enclosed UN3481 Battery Label to the box. Ship to:

Next Steps:
- Allow 3–5 business days for NerveOTX to audit your returned device.
- After audit, NerveOTX will issue an invoice for your new DX500. You will receive this via email. Your eligible trade-in credit will be applied as a reduction on the invoice. Once the invoice is paid, your new DX500 will ship out.
- If you have already purchased your DX500 unit, the credit will be issued as a refund back to your original payment method. Trade-in must be completed within 30 days of the purchase of the DX500 unit.
